- Recommend retirement communities, assisted living homes and nursing facilities.
- Arrange for relocation from the family home to a care setting.
- Refer to adult day centers and activity programs to increase brain health and socialization.
- Assure that appointments are kept and escort to appointments when needed.
- Mediate disagreements between families and care facilities.
- Act as liaison and advocate between medical professionals and the elder.
- Review and guide completion of legal and financial documents.
- Suggest home modifications to enable seniors to "age in place" safely.
