  • Visit to evaluate the current living situation. Evaluation may include a review of the health, functional abilities, mental abilities, medications, and financial and legal areas of the elder's life.
  • Create a written guide (plan of care). This document provides solutions to concerns expressed by family and the senior, and as identified in our LIFE assessment.
  • Facilitate a family meeting to air concerns and discuss options.
